Care Coordinator - Community Care Services Mildura
Community Care Services is seeking an enthusiastic and professional nurse for a permanent part-time position located at the Mildura office. The position will provide care coordination to frail older people, people with disabilities and their carers to support them to remain at home in their communities.
Applicants must have an understanding of the community aged care service system, National Disability Insurance Scheme and client directed/family focused practice. Skills and experience in aged and community care, assessment and care planning are essential. An appreciation of the brokerage models of service delivery in a community setting would be advantageous.
Classification: Registered Nurse Division 1
Salary: $46.05 per hour
Hours: 60 per fortnight
Status: Permanent Part-Time
The Integrated Care Services Division offers a diverse range of programs and services which provide high quality person centred care to patients, clients, residents and consumers throughout Bendigo and the Loddon Mallee Region. Services are provided in inpatient, outpatient, community and residential care settings including at the Bendigo Hospital site, a number of sites in Bendigo and across the Region and in client’s homes.
Our services include one of largest public community dental services in Victoria, allied health, a comprehensive palliative care service, rehabilitation and geriatric evaluation, residential care, community care packages, carer support services, community nursing, aged care assessment, transition care, residential in reach, hospital admission risk program, post-acute care, diabetes education, aboriginal services, health promotion, diversity and interpreting services, pastoral care and a large team of volunteers.
