About the role
The Community Palliative Care Service provides specialist ambulatory, community, hospital in the home (HITH) and “In Reach” palliative care to the southern sector of South Eastern Sydney Local Health District. The service also operates a Motor Neuron Disease service and all services are supported by Medical, Nursing and Allied Health Staff.
The Clinical Nurse Specialist Grade 2 (CPCiAC), is a newly created position that will expand upon the current ‘In Reach’ model of specialist Palliative Care, to improve palliative and end of life care coordination for older Australians living in local residential aged care facilities (RACFs). The CNS 2 will take an extended leadership role and will work in collaboration and receive ongoing support and supervision from the CHCK Nurse Practitioners who work primarily in the RACF space.
This newly established role will assist in the ongoing relationship between Specialist Palliative Care Services, General Practitioners, the residential aged care sector and Non- Government Organisations where appropriate, to improve access to, build staff capacity in and support the delivery of palliative and end of life care for residents and their families in RACFs.
