Right at Home is very well known and respected within the community as providing their clients with a high level of service. We are a fast growing office that is seeking a highly positive, team player to join our Perth Northern Suburbs and/or Perth West Coast offices
We are seeking an experienced, enthusiastic and highly motivated Care Coordinator to coordinate, manage and administer the delivery of care services to our clients. Our clients are a mix of Private, Home Care Package and the Disabled.
This role is responsible for:
- Conducting client care assessments and care planning
- Supervision and management of Care Staff
- Intake processing, clinical care planning and reporting
- Schedule appointments and in-home services to appropriate staff based on discipline, availability and geographical requirements
- Coordinate the provision of quality person centred care according to the client's needs, service/care plans and available funding in accordance with budget
- General administration with respect to tasks performed
- Building relationships with local stakeholders
- Positive team member
- Cert III or Cert IV (Aged/Disability Care)
- EN/RN in community nursing will be highly regarded with current AHPRA registration\
Essential
- Experinece within the aged care/disability/mental health in comunity setting
- First Aid, current CPR and Police check
- WWC (or willingness to obtain)
Preferred Skills:
- First Aid, CPR and Police Check are essential
- Experienced in assessing and planning care needs within the community
- Valid Driver's Licence
- Previous experience with rostering is highly desirable
- High level of computer literacy, with a sound understanding of Microsoft Office essential
- An outgoing personality with high energy levels, good communication skills and personable telephone manner
- Ability to problem solve and think quickly under pressure
- Possess a 'can-do' positive attitude and work well in a team environment
- Ability to multitask with a strong attention to detail while keeping calm and focused
- Excellent networking, consultation skills to develop new referral sources and relationships with appropriate external organisations
- Excellent customer service skills with exceptional verbal and written communication skills
- Sound knowledge and understanding of home care services and guidelines
- Excellent time management skills
