As a Clinical and Home Support Assessor, you will travel extensively across your region to conduct community based assessments (RAS) in clients homes. This is an exciting opportunity to bring your clinical skills and make an impact to the lives of our clients. You will be primarily based on the road, in a 'mobile' environment within our Regional Assessment Service (RAS) team, however will also be involved in any clinical projects and future clinical services Aspire4Life acquires.
Your clinical experience in aged care will contribute to your understanding of the assessments, as you bring your skills to assess the complex relationship between cognition, behavior and function in older people. You will work with the client to conduct a holistic assessment, put together a support plan and link them to services and supports in the local area.
This is a full time, fixed term position in Brisbane North, therefore candidates will need to reside in this area. This contract will end 30th June 2022.
What will I be doing as a Clinical and Home Support Assessor:
- Community based role, visiting older Australian’s in their homes. You will drive throughout your broader region, with a rostering team determining the best run to your first client and home from your last client
- Conducting holistic assessments with a focus on wellness and reablement and a client centered approach
- Capturing vital client information to build on the My Aged Care client story
- Developing support plans and identifying unique individual goals
- Connecting people with appropriate services and supports in their community. This may include assistive technology, social support or community care
Need to be/have:
- Tertiary qualification as a Registered Nurse, Occupational Therapist or Physiotherapist
- A minimum of five (5) years’ experience working in the aged care industry
- Demonstrated experience in aged care assessment and/or coordination of services
- High degree of drive, initiative, motivation, and outcome orientation
- Influenza Vaccination record
- The ability to attend up to 2 weeks of full time training (all costs covered) at our head office in Coolangatta, QLD.
- Some local knowledge and/or the ability to network with community aged care service providers and stakeholder within your region
- Comfortable travelling long distances by car
Brownie Points:
- Specific dementia or cultural awareness training
- Prior experience in promoting services through profile building opportunities (i.e. events and forums)
- Use of My Aged Care systems and National Screening and Assessment Form (NSAF)
- Current first aid certificate
