We have an exciting opportunity for an experienced Team Leader/Operations Manager in Community Health, Home Care Packages and/or Client Assessment field.

We are looking for a proactive and client centred individual with demonstrable management skills to lead the Rural North Western team to provide excellence in customer service to our valued Benetas’ clients. The successful candidate will work closely with the Regional Business Manager to achieve the regional business targets, lead the team in the successful implementation of new initiatives and continuous improvement plans.

The successful candidate will drive quality of care and responsive service delivery, with a strong focus on building an inspiring culture within the team and developing a positive staff and client experience at all times.

Key Responsibilities:

  • Effectively mentor, coach and lead the team to provide excellence in quality of care and service delivery to our valued Benetas’ clients
  • Undertake client intake and assessments, including planning, monitoring and regular reviews as needed
  • Ensure compliance to Aged Care Quality Standards
  • Actively network, promote and grow referrals to Benetas services in the Rural North Western region
  • Implement best practice quality models of assessment, care plan development, implementation, review and evaluation
  • Travel may be required to Macedon Ranges, Grampians and Hume regions.

To be successful in this position you will have:

  • Tertiary qualifications in a health or social services related discipline
  • Demonstrable experience in case management of government funded packages, community resources, and relevant legislation in the provision of aged care services
  • Demonstrable minimum management experience of 5 years
  • Demonstrable understanding of the Aged Care Quality Standards
  • Exceptional interpersonal and communication skills to demonstrate empathy and understanding of older clients and their families
  • Ability to analyse data and generate reports in Word and Excel programs
  • Competent in working with client management databases and various enterprise related processes and programs
