Casual Position with view to becoming Permanent

UPA Sydney Home Care support ageing people across the Greater Sydney area to remain living in their own homes independently. Due to current changes with-in the team, we are looking to recruit a Casual Home Care Coordinator, with view to the position becoming Permanent.

The Home Care Coordinator is responsible for the delivery of superior, Consumer focussed care and support, ensuring that the Consumers goals are met whilst maintaining their independence. You will work in partnership with Consumers receiving a home care package, to co-design a plan of care based on the Consumers preferences and assessed needs. Working within a customer service model, the Home Care Coordinator will liaise with internal and external stakeholders to ensure positive Consumer outcomes within resource/budget allocations.

You will also be responsible for the management of a small team of Direct Care Workers.

Essential Requirements:

· Tertiary qualifications in Aged Care or Allied Health

· Proven experience in the provision of Coordination, Case Management and Care Planning

· Understanding of current Aged Care Legislation, Aged Care Principles and Home Care Standards

· Understanding of Consumer Directed Care (CDC)

· Computer literate (Word, Excel, email, internet, electronic client records)

· Current Driving Licence

· Criminal History Screening

· Senior First Aid and CPR Certificate
