This position will see you planning, organising, advocating and coordinating the delivery of Consumer Directed Care through timely case management for eligible residents within the scope of Commonwealth Home Care Packages and Council’s Healthy Ageing strategy. You will be responsible for supporting an ‘age-friendly city’ through initiatives that promote community inclusion and acceptance, healthy and active living, restorative care and a broad range of appropriate services. About YouTo be suitable for this position, you will possess a tertiary qualification in Community Development, Social Work, Allied Health or Nursing with relevant industry experience. You will have comprehensive knowledge and experience of Home Care Package Programs, Commonwealth Home Support (CSHP), the Common Care Standards and the accreditation process. In addition, you will have a demonstrated knowledge of the issues impacting on aged care, as well as a strong ability to communicate with families, staff and external agencies.
