What We Are Looking For
We are looking for a self-motivated and driven Support Coordinator to help us deliver exceptional Support Coordination services to our existing clients and to drive the ongoing growth of AHS Support Coordination Services in Brisbane and the Gold Coast.
This is a key role in our expansion, and you will be the face of AHS Support Coordination in the community. You will have an active presence in the disability sector and be an experienced Support Coordinator, with an exceptional talent for building relationships with potential referrers, participants, and their families.

We are a well-established NDIS and Aged-care sector service providing a range of quality supports to people across NSW and QLD. In Queensland, our Support Coordination footprint is expanding to greater Brisbane and Ipswich.

The Ideal Candidate
- Will have an attitude of non-judgment, openness, integrity, compassion, and an eagerness to work with people of all walks of life to achieve their goals.
- have existing, well-established networks across the disability sector in Brisbane.
- Will possess exceptional interpersonal skills and be easily able to connect and develop strong relationships with community groups and organisations in the NDIS and Aged Care sector.
- Will be able to develop and implement strategies for growth and continued expansion for our Support Coordination arm of the business.
- MUST have experience or sound knowledge of NDIS and Support Coordination
- Social work, psychology or a similar degree would be highly regarded.
- Disability or Community Sector qualification for e.g., Cert IV disability, Welfare, Mental Health, or equivalent relevant experience preferred.
- Marketing and/or business development experience will be highly regarded.
- Ability to travel including own car.
- Ability to work autonomously and to self-initiate learning/research.
- Outstanding communication skills both written and verbal.
- Modern approach to disability
- Build and maintain strong relationships with key stakeholders in the care, government, and community sectors in Brisbane.
- Represent AHS in interactions with customers and key stakeholders.
- Work with the leadership team to understand objectives and set strategies in line with business objectives.
- Work with participants in the implementation and coordination of their NDIS plans’.
- Assist in identifying and accessing suitable funded and mainstream supports.
- Building capacity for participants to truly have choice and control of their lives.
- Plan review reporting and meeting support
- Be a problem solver and information gatherer for participants and families.
- Maintain up-to-date knowledge of NDIS guidelines and frameworks.
- Flexible work hours and autonomous arrangements, work from home. This role is 15 hours a week minimum to begin with, but hours will increase with demand.
