JOB DESCRIPTION

About the role:
Reporting to the Senior Care Manager, Uniting AgeWell is seeking a Clinical Care Coordinator to work within a multi-disciplinary team to contribute to, and assist with, the coordination of high quality best practice effective clinical care that supports and enhances residents' individual needs and well-being.

As a Clinical Care Coordinator with UA you will have sound knowledge of ACFI documentation and Commonwealth standards. You will provide leadership and supervision to Nursing staff and Personal Care Workers, promoting a harmonious team approach to the delivery of resident care.

Oversee the assessment, planning, implementation and evaluation of individual care
Make recommendations on the improvement of resident care, policy and procedures
Ensure that Person Centred Care and support is delivered in accordance with the residents care plans

About you:
You are passionate about what you do and want to make a difference in the lives of our residents/clients, to support them to live life to their full potential.

The ideal candidate will have:
Current registration with AHPRA as a Registered Nurse
Experienced as a Registered Nurse in residential aged care/home care
Demonstrated up-to-date knowledge and skills in Contemporary Aged Care Services
Thrive in a team and also appreciate the one to one interactions
Commitment to being part of a workplace culture based on the BEST way of life (Believe, Excel, Support and Trust)
Are willing to work within the ethos of the Uniting Church

You will need:
A current Working With Children’s Check (VIC)/Registration to Work with Vulnerable People (TAS)
Full Australian working rights
A current Police Records Check or be willing to undertake this process
A current flu vaccination and be prepared to meet this annual requirement
