ProCare Australia is currently recruiting for Carer Gateway - Contact Centre Support Worker for full time, fixed term contracts located in Mount Gravatt, QLD. The roles start early February and run until the 30th June 2021.

About the role

The purpose of the Carer Gateway - Contact Centre Support Worker is to work with carers primarily over the phone to access information, support and referrals relevant to their caring role.

The Carer Gateway - Contact Centre Support Worker is responsible for undertaking intake, assessment and planning services for carers accessing support provided by the Carer Gateway across Queensland.

Responsibilities

* Support carers with a high level of customer service ensuring carers are listened to with patience and non-judgement
* Provide Intake, Planning and Assessment services to Carers as outlined in the Carer Support Framework
* Work with carers to establish and implement agreed action plans
* Ensuring the service approach incorporates the following service principles: Carer Focussed, Practical and Flexible, Inclusive
* Creating an environment which Carers will find welcoming, real and inclusive
* Develop support relationships with carers which are positive and are built on organisation values
* Conduct Risk assessments, including assessment of suicide risk and violence risk, develop action and safety plans to mitigate any risks, providing follow up support if required, and communicating all risk with Team Leader

To Be Successful

* Qualifications and/or experience working in community services, allied health or carer/family peer positions
* Drivers License
* Appropriate IT skills
* Satisfactory National Police Records Check (within last 12 months)
* QLD Blue Card
* QLD Yellow Card
* Right to Work within Australia
* NDIS Code of Conduct
* Available to participate in out of hours activities as required to undertake the requirements of the role
* Willingness to work set shifts: 6am to 2pm or 2pm to 10pm

Salary and Benefits

The hourly rate is $33.36 + Super + Salary Sacrifice.
