Clinical Coordinator - Aged Care Assessment Services



  • Part-time ongoing role - 48 hours per fortnight

Aged Care Assessment Services (ACAS) provide comprehensive assessments taking into account restorative, physical, medical, psychological, cultural and social dimensions of the person’s care needs, using a multi-disciplinary and multi-dimensional approach, to determine eligibility for Commonwealth government subsidised aged care services and support care coordination.


Geographical area of assessment delivery is the Loddon Mallee region of Victoria, excluding Mildura and the Mallee Track areas. ACAS has offices in Bendigo, Castlemaine, Echuca and Swan Hill.


Regional Assessment Services (RAS), Shire of Campaspe. The RAS is designed to give a clear access pathway and greater choice of entry level services to Older Australians who wish to continue living a home and can do so with some support. RAS assess client needs, goals and preferences holistically and refer them to services that will help them achieve the best possible level of function and independence.


The target group for assessment are frail, older people, indigenous people over the age of fifty and for ACAS, younger people with disabilities following liaison with Disability Services.
