STATUS: Fixed Term until 10/09/2022

HOURS: Part Time 28.0 hours per fortnight

A great opportunity has arisen for a motivated individual with excellent people skills to coordinate the delivery of our valuable Planned Activity Group Program at our Lindenow outreach centre. This role is a fixed term, long service leave replacement role.

The BRHS Planned Activity Group Program provides meaningful activities to clients promoting socialisation, community inclusion and independence. Activities are focussed on meeting individual needs using person centred care planning and identified goals.

As a PAG Coordinator, your role will include supporting the development and delivery of a comprehensive activities program, providing supervision and leadership to PAG staff and volunteers and working collaboratively with the PAG Manager to review participant care plans, perform data collection and other work plan activities. The successful applicant will enjoy a rewarding role offering great hours, a happy environment and strong support from close-knit teams.

The ideal applicant will demonstrate:

  • Minimum Certificate III in Individual Care/Disability. Diploma preferred.
  • Two years team leader (or equivalent) experience in Aged Care or Disability services
  • Knowledge and understanding of the application of HACC, CHSP & NDIS- Planned Activity Group Programs for eligible participants
  • Proven understanding of contemporary principles of service delivery such as the Wellness and Reablement Model of care and Person Centred Care
  • Highly developed interpersonal skills including exceptional communication (verbal and written),
  • Good organizational and time management skills that enable efficient prioritisation of and management of competing priorities
  • Demonstrated understanding of dementia and experience in the planning and delivery of programs for people with special needs
  • Computer literacy including the use of Microsoft Office and data collection & reporting experience
