JOB DESCRIPTION

Willows Care Pty Ltd is looking for an experienced Disability Support Officer to serve our clients various needs. As a registered NDIS provider, Willows Care provides disability and aged care support services through connecting people with their communities.

Main duties

  • Assessing clients' needs and coordinate support programs needed on a case-by-case basis
  • Interviewing clients or relatives to understand the extent of support needed
  • Monitoring and reporting the progress of clients
  • Facilitate participation in community, early childhood support, development of life skills for clients
  • Facilitate group activities focusing on mental and emotional health of the clients
  • Plan and implement socially inclusive programs and projects for the betterment of clients.
  • Prepare surveys for submissions for funding and resources and provide reports to government authorities
  • Providing overall assistance to the wellbeing of the clients
  • Liaise and correspond with key stakeholders
  • Assist with end of month reporting requirements
  • Assist with regular and ad hoc duties as required

You will possess:

  • Strong knowledge and experience working within the disability sector and supporting clients with complex behaviours and high intensity support needs
  • A passion for supporting people with disabilities to achieve the best they can – bring your special talents to their experience!
  • Excellent communication skills; ability to prevent and resolve issues and to build and maintain positive relationships with all stakeholders
  • Ability to advocate and promote community inclusion and person centred support that exceeds standards and is delivered within the available resources – be creative!
  • Effective written communication skills and computer literacy in order to complete appropriate documentation
  • Resourceful and innovative approach to creating opportunities for those we support – make social change happen
  • Experience in delivering on individualised plans
  • Excellent time management skills and the ability to think laterally
  • Ability to work as part of a team and independently
  • Commitment to exceptional service delivery and a demonstrated professional approach to work

Experience requirements

Diploma level qualification in similar field

At least three ( 3) years of experience in similar industry

Annual Salary: $53,000 + superannuation

Position: Permanent full-time
