We’re inviting internal applications from skilled Community professionals with strong leadership capabilities to oversee care delivery across our SEQ South Community Services!

Overseeing a team of Care Coordinators and care staff, you’ll work to provide outstanding care to clients in your services, advocating individualised care that enhances and promotes their quality of life. Reporting to your Community Services Manager, you’ll empower your team through learning, support and leadership in order to advance the level of service to your clients.

Working closely with your Community Service Manager, you’ll ensure care governance and quality compliance are achieved through robust processes that meet internal and external auditing standards.

You’ll ensure the sustainability and growth of the service through your diligence with Home Care Package funding, CHSP funding, care planning and community engagement. You’ll plan and implement care initiatives centred on quality improvements, driving positive changes within your services.

What you’ll need:
As an experienced leader, you’ll hold considerable knowledge of community care with practical experience leading multi-disciplinary teams within an aged/community setting. You’ll have a solid understanding of the industries quality standards, frameworks, funding and how these have shaped care delivery for aged care & community services. You'll have:

Qualifications in a health, community service or other relevant discipline or equivalent community sector experience
Excellent written and verbal communication skills with proven leadership, coaching and development skills
Sound experience in management of budgets & financials
Ability to routinely problem-solve complex clinical scenarios
Evidence of innovative practices, through quality or service improvements
