You will serve as a critical link to identifying the health care needs of the individual patient and promote health and wellbeing.You will be responsible for the provision of primary health care, triage and assessment, coordination and delivery of nurse-led clinics, health promotion activities, liaison with the interdisciplinary team, care planning and reviews, conducting health assessments, providing immunisations and chronic disease management. Additionally the position will work with a number of Multiple Disciplinary Teams, including Ophthalmology, Radiology and Podiatry.The successful applicant(s) will be expected to work a 7 days rotating roster.This position requires a Working with Children Check (WWCC) issued by the Office of the Children's Guardian.
