About the Role The CHSP intake officer is responsible for managing client referrals from the My Aged Care Service Provider Portal. initial contact with Client, the preparation of Client Care Plans, completion of OH&S documentation and confirmation of Commonwealth Home Support Plan (CHSP) Services. The position will report to the Home Care Services Team Leader Victoria. Responsibilities include, but are not limited to Manage client referrals and update service information via the My Aged Care Portal Complete the AHC Intake process for CHSP referred clients Manage the central CHSP client log Needs-based client-centred phone Assessment, ensuring client choice and preferences Access information for clients, family etc and mail out to clients Appropriate referrals Strong focus on wellness, reablement and restorative care on a short-term basis Accurate record keeping for reporting requirement Attend to any other duties as directed by Team Leader Adhere to per Arcare Home Care policy and process Handle sensitive information in a confidential manner Data entry on Care Link+ to ensure client and staff records are up to date and complete Complete OH&S inspections and reports that will ensure services can be delivered in a safe environment Skills and Experience Relevant qualifications and experience in community aged and/or disability services. Demonstrated knowledge and experience working within the area of Community Aged Care Experience, knowledge and understanding of the CHSP programme and the Wellness and Reablement support is highly desirable Extensive experience in the preparation of client care plans Competence in computer-based data processing, word processing and spreadsheets. Experience in conducting Occupational Health and Safety inspections and associated documentation An excellent knowledge and understanding of the service sector and the role and operations of Local Government A thorough understanding of wellness and reablement principles Ability to work autonomously with strong communication and reporting skills Effective communication skills, reliable, flexible, and a team player Current drivers’ licence Culture Arcare is a family owned business therefore open and respectful relationships is the most important thing that we do.
