JOB DESCRIPTION

Conduct Home Support Assessments to determine a person’s needs to keep them living independently at home.


Your new company
A progressive local government organisation within a culturally diverse municipality. The area is a mix of residential and industrial areas. the organisation are currently seeking a Home Support Assessor (Aged Care) to join their team full time on a 6 week temporary basis.

Your new role
You will work with individuals, carers and families to develop a support plan to reflect the individuals aged care needs, goals and motivations including referrals to Commonwealth Home Support Program funded and non-funded services. Your responsibilities will include;

  • Conduct Home Support Assessments using the National Screening and Assessment Form (NSAF) within the My Aged Care (MAC) Portal.
  • Match and refer clients to provider(s) using the online MAC Service Finder for services under CHSP and to other services outside of the system as required.
  • Utilise the NSAF and Guidelines to determine whether a client at risk of vulnerability and will require linking support and providing linking support/short term case management for clients with high levels of vulnerability and multiple care needs.
  • Ensure all documentation is actioned and maintained in accordance with requirements of the Regional Assessment Service.


What you'll need to succeed
In order to be successful in this position, you will be an experienced Social Care professional with strong Case Management and Care Coordination skills. In order to be considered for this position, you will also have;

  • Qualifications in Social Work, Psychology, Allied Health profession or a relevant tertiary qualification and relevant experience working with aged clients, or clients with a disability.
  • Experience, knowledge and understanding of Client Centred holistic assessments, and the Commonwealth Home Support Program.
  • Experience or demonstrated understanding of the My Aged Care Gateway.
  • A drivers licence and your own vehicle.

This is a full time position based in Melbourne's inner northern suburbs. You will be remunerated between $45-50 per hour + super based on experience and will be paid on a weekly basis.
