Information about the Healthlinks Project

  • The Victorian Department of Health and Human Services has developed HealthLinks: Chronic Care. This is an alternate funding model that enables health services to use funding more flexibly for a specific cohort of patients with complex health needs to deliver services that better meet their needs.

  • HealthLinks is currently being trialled with ten Victorian public health services. Melbourne Health is a member of the HealthLinks collaborative and a control site for the trial evaluation.

  • DHHS is partnering with the Commonwealth Scientific Industrial Research Organisation (CSIRO) and health services to evaluate the HealthLinks: Chronic Care trial.

  • The primary aim is to determine whether flexible funding enables health services to develop and implement alternative models that provide better experiences and outcomes for patients with complex and chronic conditions at equal or lower cost.

The Healthlinks Improvement Project is specifically directed to improving the identification of risk factors for avoidable hospital admissions and identify service enhancements to provide timely care coordination across acute, sub-acute and community based services.
