Position Summary:

Cancer services is currently recruiting in a permanent full time Haematology Cancer Care Coordinator. Cancer services at the Kinghorn Cancer Centre is seeking a highly motivated Registered Nurse with appropriate knowledge and skills to join our friendly and supportive team as a Nelune Cancer Care Coordinator. The successful applicant will be working across a range of clinical specialties in multidisciplinary teams, in environments that are evidence-based and patient-focused. Key activities for these positions, especially for patients with complex cancer care needs, include:

* Facilitating the continuity of patient care
* Awareness of consultations, treatment plans and treatment outcomes
* Facilitating treatment scheduling
* Guiding patients and carers to information and services
* Participating in multidisciplinary care team meetings and communicating outcomes to members of health care teams
* Monitor the implementation of care against the patient care plan.

Selection Criteria:

* Personal integrity and demonstrated commitment to the mission and values of the Mary Aikenhead Ministries and St Vincents Health Australia.
* Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency
* Relevant post-registration qualifications and at least three years experience working in the clinical area of their specified post-graduate qualification
* Effective written, verbal communication and interpersonal skills.
* Demonstrated ability to work autonomously and within a team.
* Demonstrated ability to deliver and evaluate quality patient care and knowledge of the quality improvement framework.
* Competence in hospital IT systems and databases, as well as Microsoft Office products (Word, Excel, PowerPoint).
* Demonstrated ability to undertake and facilitate nursing and collaborative research projects and activities.
* Relevant post-registration qualifications and at least three years experience working in the clinical area of their specified post-graduate qualification.
