As part of our team in Outer East Region, EACH is looking for a number of part-time Home Care Workers to join us. In turn you will support the delivery of high-quality customer centric services to assist our customers maintain their independence and enabling them to lead an enriching and fulfilling life.nbsp;

What yoursquo;ll be doingnbsp;
In line with your commitment to customer care, you will have a proven ability to communicate effectively, allowing you to create positive on-going relationships with customers and team members. You will be responsible for implementing individual care or service delivery plans allowing them to achieve identified goals such as cooking, nutrition, social engagement, personal care and budgeting nbsp;

You will be working a regular roster of fixed hours (Monday to Friday only) across a couple of sites to service clients in different geographical areas.nbsp;

Who yoursquo;ll be working withnbsp;
Reporting into the Team Leader ndash; Older Adult Services, you will be part of a wider team of service coordinators and case managers to effectively support your customer#39;s needs. Itrsquo;s a team full of dynamic and motivated individuals who are passionate in supporting the health and wellbeing of those effected in their communities. nbsp;nbsp;
