The CCLHD Social Work Department is seeking a Level 1-2 Social Worker to join the team at Woy Woy Hospital on the Geriatric Rehabilitation Unit. The GRU accommodates elderly patients requiring interdisciplinary restorative care following a range of injuries, surgery or illness

This is a full time temporary position until 1/9/2021 which will then convert to part time temporary until 30/6/2022.

The Social Worker at Woy Woy GRU primarily provides counselling, assessment and complex planning for discharge to assist patients admitted to the hospital and their carers. This position works closely with the interdisciplinary team on the ward. Examples of referrals include, adjustment issues, anxiety, carer stress, capacity and decision making issues, NCAT Guardianship applications and processes, NDIS applications and processes, new diagnosis, complex residential aged care placement, elder abuse, domestic violence and legal issues.

Participation in the Social Work On Call roster is a requirement of this position. For new staff, this would not be until after approximately 6 months in the position and following a training program. Please read the On Call information attached to this position description.

This recruitment may be used to fill future positions via an eligibility list for permanent, full time, part time, temporary and casual positions.

What you'll bedoing

The purpose of this position is to:
  • To provide a professional social work service to patients and their family/carers to ensure the provision of optimal health outcomes for patients.This position will work within a dedicated interdisciplinary team and with the other inpatient social workers to deliver high quality care in a dynamic team environment.
  • Primarily the position is to provide assessment and counselling to assist patients and their carers to deal with the emotional impact of illness and to undertake case planning, liaison and advocacy and referral to ensure access to appropriate community services. The social worker is to work closely with interdisciplinary colleagues and senior managers within the hospital to ensure the development and delivery of effective and pro-active social work services. The social worker will be expected to be active in the areas of service development, service evaluation and staff and student teaching.
