The Organisation

Opportunity to join a long established organisation as they continue to expand their community support services through the delivery of personalised services. Providing a diverse range of services tailored to each individual's circumstances, this organisation improves the quality of lives of their clients through a variety of initiatives including non-clinical referral support to assist clients to stay in their homes for a long as possible.

The Responsibilities

Working from a list of eligible clients in the under 65 years age group, you will be responsible for undertaking outbound calls to assess support needs. Following an established process you will be undertaking a non-clinical community care needs assessment covering in-home and community connection requirements. Once completed you will be providing options and details of the most appropriate service providers to provide the support services. Duties include:

Liaising with clients over the phone and managing complex calls
Needs assessments
Appropriate referral partner selection
Supporting client's understanding of available support
Maintaining detailed progress notes
Ensure best practice handling of client information
Complete administration tasks as required

Your Background

If you describe yourself as a person with exceptional communication skills, strong empathy, understanding and the ability to build rapport quickly over the phone then you have the right 'soft' skills we are seeking. In addition you will possess;

Experience working in community services, disability, home care, aged care or similar sectors
Understanding and experience of assessments and referral processes provided as part of a telephone information, support process
Prior experience in undertaking needs assessments essential
Knowledge of community care and health systems and other programs
NDIS experience advantageous but not essential
Computer literacy including accurate typing and use of CRM and Excel
Current Police check or ability to obtain
