About The Role

This is a great opportunity for a process driven Clinician to work in a leading Aged Care organisation! We currently have a part time position available for someone with great knowledge about ACFI, working a variety of day shifts.

Duties & Responsibilities

  • Assisting in the resident assessment process
  • Maintaining and conducting ACFI Assessments while working closely with both Senior Management and Allied Health Professionals
  • ACFI assessments, maximising funding and reviewing ACFI processes to achieve best outcomes
  • ACFI documentation, ensuring all folders and trackers are accurate and up-to-date
  • Enter ACFI data into computer system and monitor expiry dates, annual reviews and new admissions
  • Maintain current ACFI knowledge and share information with the continuous delivery of training and education to all care staff- this guidance and support will focus

Skills & Experience
  • Demonstrated understanding and knowledge regarding the ACFI framework
  • Proven experience in achieving ACFI targets
  • A Registered Nurse with current AHPRA Registration
  • Experience using a computerised Care Management System (desirable)
  • Outstanding computer literacy

Mandatory Requirements For All Candidates
  • Evidence that you have had the 2020 Flu Vaccination or willingness to complete this before employment can be offered
  • Proof of the right to work lawfully in Australia
  • Valid National Police Check (Full disclosure or standard disclosure)
