The Role

Our Community Services team is currently seeking a fixed term part-time Client Care Officer to join our team, with a focus on providing client coordination of community care services for people with disability and frail aged people who reside in the City of Stirling. The position requires an efficient and organised work ethic as well as someone who is compassionate, able to understand our clients, and is experienced in communicating with a variety of stakeholders in a community setting. They will work in cooperation with clients and their families as well as other service providers and health professionals to coordinate support at home and in the community.

If you are looking to make a positive difference for people with disability and older people living independently in the community, this is the role for you. The position involves:

Conducting home visits to discuss clients’ care needs and level of support.
Developing and reviewing clients’ care plans to ensure their needs being met.
Implementing support services in response to assessed needs
Liaising with family, carer and other community and health professionals
Working autonomously and using sound judgement
Exercising finely tuned decision making skills in managing complex issues and changes in clients’ circumstances.
Using high level organisational and time management skills

In addition, to be considered for this role, you’ll possess the following skills and/or qualifications:

Certificate IV in Aged Care/Community Care or equivalent experience
Experience working with people with disability, the frail aged and their carers in a community setting.
Being an enthusiastic team member with strong interpersonal skills
Have an understanding of the needs of this client base living in the community
Intermediate skills using MS Office Suite (Word, Excel, Outlook, Powerpoint)
‘C’ Class Driver’s Licence.
Current First Aid Certificate.

This fixed term part-time role is for 2 days/15.2 hours per week and is offered until 30 June 2021. It is a level 5/6 Inside Workforce Agreement role, attracting a part-time salary ranging from $27,154 to $31,620 per annum (full time equivalent $67,886 to $79,052) plus superannuation.
