About the Role and You

This Grade 2 Occupational Therapist - NDIS and Subacute Services position is an exciting role working in the area of Neurological Rehabilitation with a key focus on supporting our NDIS participants to successfully navigate the NDIS pathways and facilitate complex discharge planning. Your work will also focus on supporting the Occupational Therapy and inter-professional teams in building capacity and knowledge in respect to navigating the processes of NDIS access, planning and implementation for our patients. This role will work closely with our NDIS and Complex Discharge Lead to provide guidance and advice to Occupational Therapists, and treating teams, on matters relating to the discharge and transition of clients in relation to NDIS requirements.

This role will work with existing teams and is accountable for the delivery of safe, efficient and effective evidence-based Occupational Therapy services to patients/consumers, carers and the community with a focus of building workforce knowledge, skills and capabilities to better the health outcomes of people with complex disabilities, including complex and chronic health conditions.

You will be a member of a dynamic Occupational Therapy and Allied Health team working in a collaborative model of care across Monash Health. The position works closely with the internal and external stakeholders to provide care founded on current evidence-based practice for patients accessing care through the inpatient and community based setting.

Experience and knowledge of the National Disability Insurance Scheme is highly desirable.
