Colac Area Health currently has an opportunity for a Lifestyle Coordinator to join our busy Aged CareTeam .

The successful applicant will have;

• Qualifications & Experience – Specialist skills and knowledge
• Certificate IV in Community Service – Lifestyle and Leisure or working towards same OR
• Certificate 111 in Aged Care and prepared to work towards Certificate IV in Community Service – Lifestyle and Leisure
• First Aid certificate work place level 2.
• Computer literacy particularly in Microsoft office software.
• Fit for work
• Ability to work with all members of the care team, residents and their families to assist in maintenance of dignity, functioning, self-esteem and individual worth.
• Well-developed interpersonal skills.
• High level written and other communication skills.
• Understanding and compassion for older people.
• Ability to work with families and the wider community.
• Ability to work as a member of a multi-disciplinary team.
• Well-developed skills in planning and tailoring life enhancement programs to suit individual needs.
• Knowledge and understanding of relevant legislation

The successful applicant will be required to undertake a Victorian Police Check and complete a Statutory Declaration as per the Aged Care Principles prior to commencement of employment. Some positions will also require a WWC Check.
