About the Role…
Reporting to the Managing Director, and working in partnership with the management team, the Client Services Coordinator (casual) is responsible for ensuring the delivery of high quality, client centred services and support.
Your key responsibilities will include:
- conducting comprehensive client assessments to identify health and psycho-social issues and risks for clients, and the provision of client-centred care planning to identify barriers to self-management and independent living
- the provision of case management support to clients
- making referrals to services to address client issues and coordinate the client’s care as part of a multidisciplinary team
- managing a case load through the provision of services in clients’ homes
- providing support to ensure the service meets all accreditation standards
- maintaining accurate and timely case notes and reporting on client progress
- providing effective leadership to employees under your direction
Your ability to coordinate and manage varying expectations and daily work flows will be highly attractive for this position.
About You…
You are someone who is client focused and has strong interpersonal skills; understanding our clients and operating environment, along with a personal drive to deliver performance against goals.
In particular, you will have:
- experience working in the community services, disability, home care and/or aged care sector
- an understanding of the NDIS, Disability and Aged Care requirements
- relevant qualifications
- experience in the delivery of high quality services and outcomes for clients in a outreach, home based services environment
- experience in generic care needs assessment, care planning and care coordination
- strong leadership skills with the ability to motivate staff to achieve organisational goals
- excellent communication skills, including report writing and Care Plans
- strong skills in the use of Microsoft products, and the ability to learn new systems and programs
- the ability to work flexible hours and weekend work as required
