An exciting opportunity exists for a highly motivated and experienced nurse with a strong interest in and commitment to Aged Care to join the very experienced and dedicated Nursing and Midwifery team at Hornsby Hospital. The hospital is currently undergoing redevelopment including a purpose built Dementia and Delirium unit and the Aged Care CNC will assist in the development of contemporary models of care for Aged Care patients across the hospital.

What you'll be doing
The Aged Care CNC role is designed to identify and assess patients who are at risk of dementia and delirium and develop robust care plans that are patient focussed and individualised. The CNC will provide education to all members of staff on all matters that relate to Aged Care patients to grow the knowledge and care provision of the nursing workforce to Aged Care patients.
