About The Role:

The primary objectives of the Care Advisor (CHSP – DAPCSSI) are to plan, co-ordinate and provide day to day administration of Commonwealth Home Support Programme (CHSP) in-home/individual services for consumers and ensure the activities are operated effectively and efficiently to meet the identified needs of frail elderly people & their carers.

Key Responsibilities include, but are not limited to:

· Access My Aged Care portal to retrieve referrals in order to provide in-home/individual services namely Domestic Assistance, Personal Care, Social Support Individual

· Liaise with Regional Assessment Service assessors regarding referrals if needed

· Initial and ongoing assessment of service users’ needs

· Development, implementation and monitoring and review of service user care plan in consultation with client

· Provide case management support to service users that have a person-centred focus.

· Making referrals, attending case conference, provision of information and support to service users/carers and to obtain their feedback

· Liaise with brokered service providers, if needed, to ensure services adequately meet the needs of service users

· Ensure data integrity accurately and timely for reporting and invoicing.

Qualifications and Skills

About the Successful Applicant

The successful candidates should possess but not limit to the following:

Essential:

· Minimum 3 years working experience in aged or community-based services for older people

· Tertiary qualifications in aged, disability or related area e.g. Nursing, Community, Health, Welfare or attained Certificate IV in Care Support Services / Aged Care, case management

· Demonstrated ability to work independently and work well with others

· Excellent interpersonal and time management skills

· Demonstrated knowledge and application of WH&S, EEO and anti-discrimination legislative requirements

· Speak fluent English/Cantonese/Mandarin and good command of English and with good report writing skills

· First Aid Certificate or willingness to obtain one

· Current full driver license

· National Police Clearance

· Australian Citizenship / Permanent Residency.

· Computer literacy including knowledge and competency in intermediate Microsoft applications.


Desirable:

· Experience in Home Care Services /dementia care /respite care

· Experience in case management

· Experience in leading a team

· Experience in working in a community-based setting

· Experience in working with Culturally and Linguistically Diverse populations

· Ability to speak/understand Vietnamese
