The Health Independence Program (HIP) at Werribee Mercy Hospital specialises in best practice in-center and home based programs to help clients living within the Wyndham catchment improve their health and achieve their rehabilitation goals. HIP consists of five services, including Complex Care, Community Based Rehabilitation, specialist clinics: Falls and Balance and Continence and Residential InReach.

We are seeking a dynamic CNC-Diabetes to join our team of specialist nurses and allied health staff. Your role would be to assess and develop diabetes self management plan in consultation with the client. You would work in collaboration with either care coordinators or allied health staff in supporting the client. This role often acts as a liaison between medical staff and clients to ensure that the best possible care is received and the client achieves their goals.

  • 0.42 EFT, permanent position
  • Ensure performance within the team is aligned with departmental and organisational objectives
  • Providing evidence based care to HIP clients living with Diabetes
  • Working collaboratively with the team to promote a culture of client centred care which encourages relationships and communication within the team and with client/ carer


  • Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA), holding a current practicing certificate with current AHPRA registration
  • Graduate Certificate in Diabetes Education
  • Credentialed Diabetes Educator (CDE)
  • Current Full Victorian (or equivalent) Drivers License
  • Strong, relevant clinical skills and experience in sub-acute/ community setting
  • Experienced and knowledgeable in the management of people living with Diabetes
  • Demonstrated agility to change and amend work priorities and goals accordingly.
  • Demonstrated ability to work within a multi-disciplinary team environment


  • Experience and knowledge in the management of people living complex, chronic ,physical and psychosocial care needs
