An exciting opportunity exists for an experienced nurse to take on this important position within the Cooinda Lodge Aged Care facility. As a key support person to the Manager, the Care Coordinator sets expectations, develops plans and manages process to assess, improve and maintain the care management, clinical, educational and support activities that impact care of the residents. The Care Coordinator will also deputise for the Manager.
This is a permanent full time position based at Cooinda Lodge Aged Care in Warragul- adjoining the Warragul Hospital.
Required skills and experience include:
· Current registration with the Nursing and Midwifery Board as a Registered Nurse
· Minimum 5 years post-registration experience in aged care.
· Experience in planning care for the aged
· Strong understanding of the Aged Care Accreditation System
· Hold or working towards a Post Graduate qualification in Gerontic or Aged Care Management
· Previous experience within a hospital or Aged Care Management.
· Previous experience with and understanding of the Aged Care Funding Instrument
