The Position
Title: Residential In Reach Nurse
Department: Nursing
Classification: Nurses & Midwives EA, Grade 4A Year 1 YY7
Hours per fortnight: Fixed term full time then Optional 0.2 EFT Permanent part time
Status: Fixed term/ Permanent
The Residential In Reach nurse is responsible for the effective management of Residential Aged Care Services (RACS) presentations to Echuca Regional Health (ERH) Emergency Department (ED). This dynamic, self-motivated clinical liaison nurse will provide a consultative service, prompt assessment and short term case management to the RACS clients, staff and GP’s reducing the presentations to ERH ED. The RIR reliever will be available for all RIR leave replacement (ADO, Personal Leave, Annual leave and Long Service Leave).
Applications are sought from suitably qualified Division 1 Registered Nurses with previous experience in Aged Care Services and Acute Nursing. They will have advanced clinical skills and knowledge to work independently in the role and join the IPAC Department at Echuca Regional Health.
Key responsibilities include:
- Reduce avoidable presentation of RACS patients to Emergency
- Provide an alternative model of care to an Emergency presentation
- Improve support for RACS staff
- Work with RACS to plan for adverse clinical events
- Foster collaborative relationships with ED staff, GP’s and RACS staff to develop coordinated management plans
- Develop, evaluate and market the RIR service model of care in the hospital, RACs and Community agencies, engaging with RACS to develop a yearly Aged Care Forum.
- Advocate, promote and assist the clients and families with Advanced Care Planning.
- Provide accurate, quarterly statistical analysis of the RIR : VINAH data, preventable admissions to ERH and diagnosis management summaries. Forming ongoing feedback and education to RACS from this data.
- Provide relief for all leave replacement.
Our team requires someone who has:
- Qualifications in Aged Care and/or Graduate Certificate in Advanced Nursing skills desirable. Minimum of four years post graduate experience.
- Well-developed interpersonal and written communication skills.
- Demonstrated knowledge of RACS systems, ability to work autonomously and effectively within a multi-disciplinary health care team.
- Strong leadership skills and the ability to be self-motivated.
This position requires completion of a satisfactory National Police Check and (where relevant) provision of a Working With Children Check and check on the Disability Worker Exclusion Scheme prior to appointment.
