Care Coordinator - Community Care Services (Bendigo)

Seeking an enthusiastic & professional Care Coordinator for our Bendigo Office
Providing care coordination to frail older people, those with disabilities & their carers to remain at home
Understanding of the Community Aged Care system & NDIS is essential
Opportunity for secure & ongoing employment (part-time @ 64 hours per f/night)
Salary as per EBA

Applicants must have an understanding of the community aged care service system, National Disability Insurance Scheme and client directed/family focused practice. Skills and experience in aged and community care, assessment and care planning are essential. An appreciation of the brokerage models of service delivery in a community setting would be advantageous.

01.Tertiary qualifications in health or community services essential

02.Substantial experience in the areas of aged and community care , including people with disabilities and their carers

03.A sound appreciation of the key concepts of assessment, care planning, case management and brokerage models of service delivery in a community setting

Essential:
04.Demonstrated ability to problem solve in a variety of situations

Essential:
05.Highly developed communication and interpersonal skills, both written and verbal, including liaison and negotiation skills

Essential:
06.Sensitivity to the needs of clients from diverse backgrounds

Essential:
07.Well-developed skills using computers including the Microsoft office suite and data base programs

Essential:
08.Understanding and skills in budget management
