About the Role

The Social Work Department is looking for an experienced Grade 1 social worker to join the team working in the sub-acute setting. This position is a permanent full time position and is part of a flexible team who collaborate to provide a holistic approach to service delivery. This clinical role provides an opportunity to showcase your skills in working with people from a range of diverse backgrounds as well completing complex psychosocial assessments and development of patient centred care plans. Sub-acute services offers specialty inpatient rehabilitation and geriatric evaluation and management, focusing on older people who have reduced functional independence.

You will be a member of a dynamic Social Work and Allied Health team working in a collaborative model of care across Monash Health. The position works closely with the internal and external stakeholders to provide care founded on current evidence-based practice for patients accessing care through the inpatient and community based setting.

Skills required include experience in:
Psychosocial assessments, risk assessments, patient and family sensitive model of care

Discharge planning for complex case loads

Team approach to managing risk and vulnerable people and their families

About You:
Experience working as a social worker, experience in health and working with multidisciplinary teams,

Demonstrated experience as an autonomous multidisciplinary health team,

Possess excellent written and verbal communication skills,

Experience with acute and chronic healthcare situations supporting vulnerable people demonstrate,

Utilisation of the multidisciplinary approach to vulnerable people living and accessing health services in a rapidly growing geographical location.

What you need:
Bachelor Social Work or Master Social Work (Qualifying)

Experience working in health setting

Current driver’s license

Current working with children’s check
