Beata Homecare provides home care services to support the independence of elderly and disabled clients from various cultural backgrounds in Melbourne.

We are currently looking for a Case Manager with a nursing background to join our team on a full-time contract.

You must be a registered nurse for this role.


You will be responsible for providing consumer-centred care coordination to your consumers through the following:

  • Undertake assessments
  • Develop, implement and evaluate care plans
  • Coordinate support services
  • Engage with external stakeholders
  • Manage complex and challenging situations
  • Undertake administrative support
  • Undertake quality assurance activities
  • Manage home care package funds and budget
  • Provide support to direct care workers

You will report to the Program Manager.

You will be based at our office in Braeside, Victoria.


  • Must be registered as a nurse with AHPRA
  • Experience in Aged Care or Community Services Sector
  • Case Management or Team Leader experience desirable
  • Good computer skills
  • Excellent written and verbal communication skills
  • Problem-solving skills
  • At least 2 professional reference

Must be able to provide originals or certified copies of the following documents:

  • Victorian Drivers Licence and reliable vehicle with insurance
  • Police Check (within 3 months)
  • First Aid Certificate (within 3 years)
  • CPR Certificate (within 1 year)
  • Working with Children Check
