Beata Homecare provides home care services to support the independence of elderly and disabled clients from various cultural backgrounds in Melbourne.
We are currently looking for a Case Manager with a nursing background to join our team on a full-time contract.
You must be a registered nurse for this role.
Role:
You will be responsible for providing consumer-centred care coordination to your consumers through the following:
- Undertake assessments
- Develop, implement and evaluate care plans
- Coordinate support services
- Engage with external stakeholders
- Manage complex and challenging situations
- Undertake administrative support
- Undertake quality assurance activities
- Manage home care package funds and budget
- Provide support to direct care workers
You will report to the Program Manager.
You will be based at our office in Braeside, Victoria.
Requirements:
- Must be registered as a nurse with AHPRA
- Experience in Aged Care or Community Services Sector
- Case Management or Team Leader experience desirable
- Good computer skills
- Excellent written and verbal communication skills
- Problem-solving skills
- At least 2 professional reference
Must be able to provide originals or certified copies of the following documents:
- Victorian Drivers Licence and reliable vehicle with insurance
- Police Check (within 3 months)
- First Aid Certificate (within 3 years)
- CPR Certificate (within 1 year)
- Working with Children Check
