What You Will Be Doing
The position will provide outreach services in community settings, residential aged care facilities and in client’s homes. The RN works competently within their scope of practice to deliver safe, evidence based person centred care to achieve optimal health outcomes or a dignified death.
It involves facilitating collaborative integrated care planning across acute and community based services. The role operates within a multidisciplinary team, to coordinate and provide expert clinical service, in line with relevant standards, guidelines and the policies of Peninsula Health.
What You Need
We anticipate you will have:• Registered Nurse with current Nursing and Midwifery Board of Australia registration.
• Experience and knowledge regarding chronic disease management• Demonstrated commitment to delivering high quality outcomes for consumers.
• Demonstrated experience in working both independently and as a part of a multi- disciplinary team with the ability to work effectively, supportively and flexibly.• Demonstrated experience working with at-risk and vulnerable people with complex needs.
*Current Victorian Drivers Licence. Working with Children Check and Police Check
