What you'll be doing
The Access and Referral Centre (ARC) is the single point of access for referrals to ISLHD Health Services. The Enrolled Nurse/Intake Officer will provide timely information and process referrals. Effectively this will assist patients, staff and community to navigate the health service system, and enable more effective transition across the Acute/ Community/ Aged Care Interface. The ARC promotes a “no wrong door” philosophy.
The position will also be responsible to facilitate referrals for telephone coaching services and will provide the initial client identification, screening, registration and contact. These service elements will be conducted over the telephone. The position will be located within the Access and Referral Centre.

Selection Criteria: * Current NSW Enrolled Nurse with AHPRA
- Experience in undertaking comprehensive assessment
- Extensive experience working across the acute/community interface and an understanding of the issues encountered navigating services across the continuum of care
- Effective communication skills which include listening, verbal, written with a customer service focus
- Ability to plan organise workloads and demonstrate decision making skills
- Working knowledge of computer based systems including CHOC/EMR, IPM and Microsoft Products
- Knowledge of ISLHD and community Based services (including Non-Government Agencies servicing the ISLHD)
- Demonstrated ability to work as a part of a multidisciplinary team
