About the Role
An opportunity exists to join the Infinite team as a Care Coordinator responsible for ensuring that all residents in the facility receive high quality clinical care. The role will have the responsibility for ensuring care needs are managed within the facility and that resourcing, documentation and assessments are in alignment with best in class clinical practices. This full time role reports to the Facility Manager, with support and guidance from the Care Manager.
Day to day duties will include, but are not limited to;
- Provision of clinical expertise and leadership of clinical and care service teams across the facility
- Assist Care Manager to lead carer teams to meet all clinical needs of residents - including monitoring and tracking clinical indicators to achieve best practice clinical care outcomes
- Responsible to manage all aspects of the assessment, documentation and submission of ACFI requirements within the facility
- Support the effective documentation and full clinical team user support of policies, procedures and IT system requirements in the facility
- Coordination of assessment, care planning and review processes within the facility
- Support the admission of new residents to facilities for optimal care assessments and initial consultations with resident and families
- Key role in ensuring accurate information is recorded and in accordance with all organisational and legislative requirements (ACFI, resident records and staff time recording)
- Input and oversight into the facility team rosters and ensuring optimal skill mix available for delivery of care to residents
- Manage and document clinical risk within the facility wing and ensure a commitment to continuous improvements within the facility
What we are looking for
- You will have an innate empathy for our residents and be committed to continually improving the quality of clinical care provided to them
- Experience and extensive knowledge of clinical assessments, care plans, ACFI and accreditation compliance and documentation
- Engaging style that has ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment
- Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA)
- Post graduate qualifications in aged care would be advantageous but not essential
- High level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements
- A confident team player with excellent communication and interpersonal skills that has a passion for leading clinical outcomes
- Ability to manage multiple stakeholders and have a balanced and level headed approach to emergency and stressful situations that arise
