JOB DESCRIPTION

The primary position objectives are to provide culturally appropriate Adult and/or Paediatric Occupational Therapy services, as directed to the Aboriginal and Torres Strait Islander community as a member of the multi-disciplinary health care team.

Our Organisation

Gladstone Region Aboriginal and Islander Community Controlled Health Service (GRAICCHS), trading as Nhulundu Health Service is an Aboriginal Community Controlled Health Service operating in Gladstone and the greater Central Queensland Region delivering a comprehensive range of primary health care, aged care and community support service functions.

GRAICCHS is a multi-faceted Human and Social Services provider that is responsible for the delivery of integrated comprehensive primary health care, aged care, health education and promotion programs, community support services and child protection system. In addition to the above mentioned services GRAICCHS provides programs under the "Close the Gap" and National Federation of Reform Council (NFRC) initiatives. There are staff and contractors employed with the organisation to ensure that the service delivery of these programs is provided within funding and statutory guidelines.

General Responsibilities

Provision of OT Services

The roles and responsibilities of this position include, but are not limited to:

  • Develop a structured work plan outlining key activities, timeframes and targets in line with job role and with the overall organisational plan
  • Develop a database of key local contacts, networks and organisations
  • Provide clinical services to clients during hours of operation of the service, in keeping with accepted best practice standards. This will include clinic-based appointments, home visits and potentially school visits
  • Therapy activities may include:
  • Review clients medical history, evaluate their situation and needs
  • Developing treatment plans with clients, identifying specific goals and the activities for goal achievement
  • Providing support and programs to achieve increased independence in tasks of daily living
  • Exercise programs for maintenance of and increasing mobility for clients with chronic conditions
  • Evaluate a client's home or workplace and, on the basis of patient's health needs, identify potential improvements and recommendations
  • Education with client, family and employer about how to accommodate and support the patient
  • Recommend special equipment, such as wheelchairs and eating aids, and provide education on use
  • Assess and record patients' activities and progress for patient evaluations, for billing, and for reporting to physicians and other healthcare providers
  • Support with stroke rehabilitation and oedema management;
  • Provide culturally responsive assessment and intervention for adults/children including individual and/or group programs, group education sessions and one on one consultations, as required
  • Provide clinical services using a multi-disciplinary and/or inter-professional approach.
  • Provide regular reports to the Manager Aged Care and NDIS as required.

Service and Professional Development

  • Commit to ongoing professional development and participate in clinical leadership and supervision sessions, identification of education needs, performance assessment and mentoring sessions
  • Work with the Senior Management Team to identify any issues arising in processes or systems within clinics which impact on service delivery, Medicare billing or safety and quality.

Operational Process & Administration

  • Liaise with Queensland Health and Education Queensland regarding services available and referral pathways
  • Ensure information is recorded accurately and in a timely manner in the electronic health record system, and that records are maintained in accordance with RACGP standards
  • Ensure maintenance of privacy and confidentiality Actively maintain knowledge of the application of the MBS relevant to your job role and ensure information about completed MBS items is effectively captured for accurate processing of claims
  • Prepare ad hoc reports as required.

Develop & Maintain Linkages

  • Foster productive working relationships with other GRAICCHS staff in the delivery of seamless support services
  • Foster productive working relationships with internal and external providers
  • Work to foster productive working relationships with relevant government departments, Queensland Health and other specialist providers
  • Develop and maintain effective relationships with the communities in the target areas

Legal Obligation - All relevant health professionals who, in the course of their duties, formulate a reasonable suspicion that a child or young person has been abused or neglected in their home/community environment, have a legislative and a duty of care obligation to immediately report such concerns to the Department of Child Safety.

Qualifications/Knowledge/Experience

  • Relevant degree in Occupational Therapy and continuing unconditional registration with AHPRA and the Occupational Therapy Board of Australia demonstrating currency of practice
  • Demonstrated ability to comply with the profession's code of conduct and ethics and function in accordance with legislation affecting occupational therapy practice
  • AHPRA registered allied health professionals do not require a Blue Card as their registration meets this requirement, however staff must declare any future charges or convictions that could potentially affect your role within GRAICCHS. (Yellow Card required for NDIS related positions).

Practical Requirements

  • Ability to work out of normal hours of duty as reasonably required from time to time
  • Current C Class Drivers Licence (Qld)
  • Ability to travel across region as required
  • Right to work in Australia

General Responsibilities

  • To be determined by the Manager Aged Care and NDIS:
  • Eg Referrals, outcomes, promotion of working relationships and community networks.

Selection Criteria

  • Demonstrated ability to work with Aboriginal and Torres Strait Islander communities and their leaders, respecting traditional culture, values and ways of doing business
  • Working knowledge of health issues affecting Aboriginal and Torres Strait Islander people and a strong commitment to improving health outcomes
  • Ability to work sensitively and effectively with Aboriginal and Torres Strait Islander people
  • Commitment to the philosophy and practice of Aboriginal Community Control
  • Demonstrated skill in working in a primary care setting, in particular the ability to work as part of a multidisciplinary team with a sound understanding of your role in the cycle of care for a client
  • Experience in providing assessment, case management and occupational therapy services to Aboriginal & Torres Strait Islander people and the greater community
  • Demonstrated effective verbal and written skills with particular emphasis on interpersonal communication skills, confidentiality and record keeping
  • Computer skills - able to work effectively with electronic client record systems
