About The Role

Clinical case managers of the Monash Health Clayton Continuing Care Team work within a multi-disciplinary team to provide treatment and support to adult clients experiencing mental health issues. Case manager’s work with an allocated client caseload to address the biopsychosocial needs of individuals on a client centred, recovery based model. Our aim is to delivery high quality care that promotes individual client's self-efficacy, independence and community integration.

You will enjoy the challenges of working with the complexities of mental health to improve individual quality of life and to make a positive difference every day. You will work closely with a range of mental health services to effectively co-ordinate client care, including inpatient units, CATT, CCU, PARCS, Mental Health Support, NDIA and Drug and Alcohol services.

What You Need to Know

The position involves Monday - Friday daytime roster. Clayton Community Mental Health Service is located close to the CBD and the South East sector. Clients are seen in the clinic as well as in their own home environment. It is an autonomous role with the balance of working in a team environment. Access to high quality clinical supervision is offered.

  • Qualifications and relevant registrations in Nursing, OT or Social Work
  • Capacity to work independently and within a team
