Join this Occupational Therapy Clinic specialised in Aged Care in providing in home and community support along with their leading services providers.
This clinic offers services across a variety of health conditions, completing assessments and developing recommendations to overcome barriers with their participants.
Responsibilities:
- Assist with trialling assistive technology and designing home modifications
- Provide services and consultations to individuals, carers and families; including assessment, intervention and evaluation.
- Maintain frequent communication with key stakeholders involved in the client’s care
- Manage care planning to assist with funding expectations for the client or care managers.
- Manage client applications and reports relevant to funding in accordance with their requirements.
- Recognise and respond to the needs and requirements of each individual client and/or carer.
- Develop and provide individualised, client focused care in conjunction to their needs, goals and preferences.
- Provide prompt and adequate resolution of client complaints
- Help to achieve goals and promote independence, safety and quality of life.
