Located in Boonah in the Fassifern Valley our Teviot Service require a Home Care Coordinator

Let’s talk about the role:

The role of our Home Care Coordinator is to support our older Australians to live confidently, safely and independently in their homes whilst maintain effective coordination of home care support.

This is a Casual role.

In addition to this, you will;

  • Support clients to develop their personal independence plan (PIP)
  • Coordinate and monitor the delivery of services outlined in the PIP
  • Monitor client satisfaction with the services and adjust the service mix to ensure a high degree of satisfaction
  • Undertake risk assessments associated with the delivery of services
  • Promote and explain the service to the potential user and families/carers involved.

Now let’s talk about you:

To be successful in this role you will have Tertiary qualifications in a relevant industry or equivalent experience in aged care.

  • Be respectful of older people, families and carers
  • A passion for promoting independence and self-determination by older people.
  • Excellent interpersonal communication.
  • Incumbent must maintain a positive federal police check and Australia drivers licence
