We have a rewarding opportunity for an experienced Case Manager to oversee a portfolio of home care package (HCP) customers.
You will be inspired by making a positive difference to the lives of our customers, conducting in-home assessments to identify their needs and developing tailored care solutions to enhance their quality of life.
Work with flexibility and autonomy, dividing your time on the road visiting our customers and networking with relevant community stakeholders, and liaising with our internal teams and ensure the smooth delivery of care services.
Skills and Experience required:
- Previous experience in case management and working in a community setting preferable
- Knowledge of the Community, Disability or Aged Care system and care packages desirable
- Excellent ability to connect and build rapport with multiple stakeholders
- Team-oriented with an open and collaborative work style
- High attention to detail and focused on achieving positive client outcomes
- Resilient and able to work effectively under pressure
- Empathetic, understanding and passionate about caring for people
- Intermediate computer skills
- National Registration with Australian Health Practitioner Regulation Agency (AHPRA) desirable but not essential
- Clinical experience desirable but not essential

To be considered for this role, you will have a qualification in health and/or community services with strong previous case management experience.
