Centacare is the social services arm of the Catholic Archdiocese of Brisbane and has been operating in South East Queensland for over 50 years.

Centacare currently provides services to over 100,000 clients per year, and currently employs over 3000 workers. A multi-faceted organisation, Centacare provides a broad base of social services including: family relationship services, disability services, community services, aged care, pastoral ministries, and child care services including outside school hours care.

Centacare Community Services directorate (CCS) is structured into 6 regions with a centralised support service delivering aged care, mental health, housing support, homelessness, respite and transition, independent living and support for people with a disability and learning and leisure services.

The job:

Centacare Community Services Support Coordination CHOICES have a Full Time 12 month fixed term contract position available for a Support Coordinator servicing Brisbane, North and South of the river. The primary purpose of this role is to work creatively with people to facilitate capacity building, support them to explore opportunities and ultimately building confidence to engage with the NDIA and its developing marketplace to pursue their individual goals.

In this role you will:

  • Identify community, mainstream, funded and informal supports to assist a person to pursue their goals.
  • Support a person by consciously and deliberately providing unbiased choice and control in all of their decision making; upholding their desired actions at all times.
  • Support a person to maintain their chosen networks – community, mainstream, funded and informal.
  • Support a person to arrange clinical assessments if and as required.
  • Support a person to engage in funded services if and as desired.
  • Support a person to develop an understanding of their NDIS funding that has been deemed reasonable and necessary in their plan to maintain supports across a plans duration.
  • Support a person to connect to and understand their online My Place Portal.
  • Have a capacity building approach at all times.
  • Think creatively to overcome barriers to engagement as they arise.
  • Be a team player and get on-board/excited with where Centacare CHOICES Support Coordination is heading.
  • Contribute to continuous quality improvement in development of Centacare CHOICES team.
  • Ability to accurately enter support coordination actions performed into a client management system as support coordination is a fee for service (we are a service provider in the marketplace too!).
  • Ability to meet Key Performance Indicators while developing customer and stakeholder relations.
  • Represent Centacare and embody its Vision, Mission and Values.
What we need from you:
  • Advanced knowledge and understanding of the NDIA and sector developments.
  • Intermediate skills of the Microsoft Office suite of products;
  • Intermediate skills in the use of TRACCS and/or other client management systems.
  • Knowledge and understanding of Disability Services Act 2006, Aged Care Act 1997 and Mental Health Act 2016.
  • Knowledge and understanding of National Aged Care and Disability Services Reforms and implications on community services.
  • Advanced communication skills and ability to adapt to differing receptive styles of communication.
  • Ability to work with minimal supervision, exercising initiative and professional judgement at all times.
  • Ability to prioritise workloads to meet schedules through high level time management skills;
  • Strong leadership qualities with a high level of tact, integrity and discretion;
  • A valid Queensland drivers licence;
  • A National Police Certificate; and
  • A current Blue card and Yellow card exemption, or willingness to obtain.
