The Position

Case Managers operate within the community to work with our clients, their families and the wider care network, allowing elderly Australians in regional and remote communities to access services of their choice. Our Case Managers are accountable for the provision of home care services and support to potential, new and current clients, carers, and families. The role enables clients to optimise their Home Care Package; including identifying the need or opportunity to optimise service or package options to ensure client is getting services best aligned to support them build independence, health, and wellbeing. They help achieve creative and focused services through responsive and considered case management. Acting as a trusted advisor case managers positively affect their decision-making to achieve high client satisfaction.

  • Permanent part time position available (4 days per week)
  • Position can be based in either Rockhampton or Gladstone
  • Be willing to travel across the Rockhampton to Gladstone region.

As a Case Manager you will be conducting person centered assessments to develop our client's care plans and budgets to provide fulfilling care and support by facilitating their choices. As well as, working collaboratively with other health professionals within the organisation and externally. As well as delivering;

  • High-quality, values-based case management for a diverse case load
  • Person-centred assessments that result in strength-based goals and outcome measurement
  • Develop networks and partnerships with important partners
  • Management of individual budgets, achieve required program outputs and report through to the Senior Case Manager
  • Promotion of services and identify opportunities to expand our portfolio of services
  • Maintaining safe practices through risk management strategies

Required skills, experience, and credentials

  • Tertiary qualifications (certificate or higher) in a relevant field.
  • 2 or more years of relevant experience in the Disability, Aged Care or Youth sector
  • Previous aged care and/or case management experience, preferably both
  • Successful outcomes in building community engagement
  • Commitment to person-centred health and wellbeing outcomes
  • Experience in guiding and motivating others to provide quality service delivery
  • To foster our culture which is based on respect for our clients
