Nursing and Medical @ Home Team is a team that delivers care in the western region community catchment at WH: this includes care in residential aged care facilities and in the community setting. Our Goal is to support patients to return or remain at home following a change in health or functional ability.
Primarily, the position is a Liaison role is responsible for referral acceptance, triaging and work allocation. They are also in the front line for providing advice around management of clinical issues.
To be successful in the role you
- Have recent clinic skills and general clinical knowledge
- Have excellent communication skills – verbal and written
- Be able to work in a busy environment – be good at prioritizing and work ethic
