Regis Caboolture aged care home is situated in a quiet residential precinct next to Caboolture Public Hospital and the neighbourhood shopping centre. Our aged care home features a beautiful central garden courtyard with a walking track and bus stop for residents to utilise. It is close to the child care centre, local schools, Caboolture Air Museum and the Caboolture Historical Village.
Our active and enthusiastic residents spend time in countless different ways. From getting in touch with relatives using Zoom, Facebook, Face time to indulging in fishing trips and Robotic Companion pets.
Regis Caboolture specialises in secure Dementia care and support for the frail aged, with the focus on meeting the needs of residents with a close personal approach.
Caboolture aged care home offers 171-beds in single and shared rooms with en suites, and contemporary furnishings and finishes.
About The Role
This is a great opportunity for a process driven Clinician to work in a leading Aged Care organisation! We currently have a position available for someone with great knowledge about ACFI, working in a permanent role.
Duties & Responsibilities
- Maintain current ACFI knowledge and share information with the continuous delivery of training and education to all care staff- this guidance and support will focus
- Enter ACFI data into computer system and monitor expiry dates, annual reviews and new admissions
- ACFI documentation, ensuring all folders and trackers are accurate and up-to-date
- ACFI assessments, maximising funding and reviewing ACFI processes to achieve best outcomes
- Maintaining and conducting ACFI Assessments while working closely with both Senior Management and Allied Health Professionals
- Assisting in the resident assessment process
- Outstanding computer literacy
- Experience using a computerised Care Management System (desirable)
- A Registered Nurse with current AHPRA Registration
- Proven experience in achieving ACFI targets
- Demonstrated understanding and knowledge regarding the ACFI framework
- Valid National Police Check (Full disclosure or standard disclosure)
- Proof of the right to work lawfully in Australia
- Evidence that you have had the 2020 Flu Vaccination or willingness to complete this before employment can be offered (VIC & QLD currently exempt)*
