JOB DESCRIPTION
TLC Nursing Agency is looking for a full-time Disabilities Services Officer who can be responsible to provide support, information and advice to clients on emotional, recreational, health, housing and other social welfare matters.
Tasks and Responsibilities:
- assessing clients' needs; provide direct support to people with a disability, on a day to day basis, according to individual needs, plans, programs and direct observation.
- developing and implementing support programs; communicate and interact with consumers, families, community agencies and staff on day to day and other matters as necessary.
- interviewing clients and assessing the nature and extent of difficulties
- monitoring and reporting on the progress of clients: maintain consumer and home records, write notes and routine reports
- assessing community need and resources for health, welfare, housing, employment, training and other facilities and services
- liaising with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and services
- supporting families
- driving clients to and from medical appointments
Skills & Experience
- Qualifications in the Aged Care industry
- Over 2 years of work experience in the industry
- An understanding of the ageing process
- A comprehensive clinical knowledge, particularly to age related health issues
- Understanding of and commitment to continuous quality improvements
- Excellent communication and interpersonal skills including demonstrated experience interacting with a wide range of people
- Ability to prioritise and manage workloads according to residents needs
- Ability to work as a member of a multi-disciplinary team
The successful candidate will receive a salary package of $55,000 - $65,000 p.a. + superannuation.
