About the Opportunity

  • Join the dedicated, multidisciplinary Specialist Community Palliative Care Team consisiting of primary care, specialist services, community based services, inpatient, aged care and volunteers
  • Deliver compassionate, individualised care for people with advanced life limiting illnesses, their families & carers
  • Provide a combination of direct/shared/consultative care across settings, in liaison with other FWLHD palliative care health professionals
