Your new company

Work with a highly reputable local council in Melbourne's Northern Suburbs. With deep ties to a highly diverse community, your work will have a direct impact on said community and thorough interaction with it's people across the municipality.

Your new role

On a 3 month contract, the Service Facilitator and Response Officer will work across two teams in service delivery and will receive referrals and enquiries regarding Aged Care and Disability services, provide accurate information about services, register new clients, and provide effective administration to support client pathway and the operations of the Aged and Disability Department. Your role will also include:

  • The role provides a customer interface for business units within the Aged and Disability Department. This involves basic rostering skills to assist with changes to services for older residents and people with a disability.
  • The development, and review of service plans for Domestic Assistance, Personal Care, Individual Social Support, Meals, Flexible respite, Shopping and Home maintenance/modification services.
  • Monitor client well-being in relation to whether service plans meet the needs of the client and are within organisational service scope, taking action such as reviews and referrals as required.
  • Knowledge of how to use client management systems such as Carelink Plus, plus an understanding of the Commonwealth Home Support Program.

What you'll need to succeed

To succeed in this role you will have relevant qualifications in Welfare, Social Work, Psychology or Behavioural Sciences along with:

  • An ability to undertake a variety of tasks with minimal supervision, including the ability to plan and organise work to meet quality standards and manage constant requests for information or action from customers.
  • Proficiency in computer skills including fast and accurate keyboard skills; capacity to learn and retain new information, including an aptitude for using data bases such as Carelink Plus.
  • Well-developed written communication skills to clearly record enquiries and requests for service and to prepare routine correspondence.
  • Demonstrated experience in developing or contributing to the development of service specific plans which provide clear expectations of service delivery for both client and support workers (Knowledge of re-ablement and client centred approaches an advantage).
  • Demonstrated ability to monitor service implementation and client well-being and identify when a service requires review or is out of program scope.
